Registration

To complete the registration process, we will need a signed medical release allowing your eye doctor to release your eye report to us, and you will need to have had an eye exam within the last 12 months. All children and adults are registered through intake and then a case manager is assigned.

SOCIAL WORK

Receiving Cleveland Sight Center Services

Whether you have been referred by your eye doctor, a friend or found us online, you must start by registering with us. This can be done over the phone or in person. Within a week, your social worker will then contact you to plan next steps. Generally, your social worker will speak with you on the phone and schedule a time to meet with you in your home. During this initial visit, they will be able to make a good determination about what, if any, vision you still have and what services are recommended. From there, your social worker will begin referring you to services at Cleveland Sight Center that will best serve your needs.

Deaf-Blind Services

We have a special Deaf-Blind Services program that is affiliated with the Helen Keller National Center for Deaf-Blind Youth and Adults. Being part of this national network helps us keep abreast of new developments in this area.

Our Deaf-Blind Coordinator provides an assessment of your needs and designs a plan of individualized treatment. Your coordinator will get to know your family, friends and any other people who have an impact on your life to give them the information and the skills they will need to help and support you.
