SCHOOL-TO-WORK TOOLKIT LENDING LIBRARY


The School-to-Work Toolkit is a lending library of assistive technology devices and software.

How to Use the Lending Library

Step 1 - Complete the Device Loan Request Form. Make sure your request meets the criteria listed below. Please note, that as the requestor you are responsible for returning the loaned item.

Step 2 - Email the completed form to info@clevelandsightcenter.org and your request will be processed. You must pick up and drop off the item to and from Cleveland Sight Center, or pay for UPS shipping. We can charge your account if you have one.

Criteria

The purpose of the loan can be to test a product and assist in decision making, to provide interim technology while waiting for device repair or funding, to provide emergency accommodation, and to practice skills learned during training. The following criteria is required for participation in the loan program:

  • The loan must serve a vocational purpose.
  • For the duration of the loan, the consumer who receives the loaned item must be actively seeking employment, be employed, be enrolled in school, in the process of enrolling in school, or receiving vocational training.
  • Loans are not made directly to individuals who are blind or visually impaired. Loans are made to agencies serving individuals who are blind or visually impaired.
  • The requestor (i.e., vocational rehabilitation counselor, case manager, teacher, or social worker) is financially responsible for the item to be loaned. If the loaned item is not returned or returned damaged, the requestor must repair or replace the item.
  • Loans are short term with a specified date of return. The return date will be related to recipient's vocational activities.
  • Because requests for loans will likely be greater than the quantity of items available for loan, requests will be prioritized in the following manner:
    1. Recipients who are in danger of losing their job due to lack of adaptive technology.
    2. Recipients who are enrolled in school, pursuing training, or participating in vocational service, but who do not own or have sufficient access to similar adaptive technology.
    3. Recipients who cannot afford, or due to their circumstance, do not qualify for a third-party payer.
